cgi form

Q:
for some reason my form stopped working. I'm using free cgi from
bignosebird.com. My form is located at:
http://www.goldstarcash.com/apply.htm

Seems that the form works as it tries to redirect me to the ooopps.html
page that does not exists.

Nothing else could I identify remotely.
