Installing a perl survey script

Hello Peter,
I downloaded this survey script from
http://bignosebird.com/carchive/survey.shtml
It's called the BNBReader Survey.
I cannot get it to work and was wondering if you don't mind taking a look
on it.
I got all the users permissions to read and write and the local to
execute, read and write.
The program consists of 4 files:
survey.htm
survey.cgi
survey.log
mysurvey.srv

After I hit submit on the mthl page is get a Data File is Missing error.
It seems like it is not finding the Data FIle (*.srv).

The file diag.cgi spits out all the server information.

I hope you get a chance to look at it and appreciate any pointers you
might have.

Most probably it is erroneous setting of the full path to the survey file.

Check that the directory where the survey file is gives write permission
to the www-data user.
